Why Some Muscles Get Weaker the More You Use Them

The Neuromuscular Junction & Myasthenia Gravis

Imagine your eyelids drooping by evening, or your arms giving out after a few easy repetitions. In some conditions this is not normal tiredness. The message from nerve to muscle is getting blocked.

The Science

  • Gilhus (2016, N Engl J Med): in myasthenia gravis, antibodies target acetylcholine receptors at the neuromuscular junction, reducing the muscle's ability to receive the nerve signal.
  • The hallmark is fatigable weakness: strength drops with repeated use and recovers with rest, often affecting the eyes first.
  • Howard (2018, Ann N Y Acad Sci): antibodies can also trigger the complement system, damaging the junction and worsening signal loss.

The Protocol

  • Learn the pattern: weakness that worsens with use and improves with rest.
  • Watch for early eye signs such as drooping lids or double vision.
  • Do not treat true, fluctuating weakness as something to push through.
  • Seek a medical evaluation for an accurate diagnosis.
